The problem

The overseas visitor manager needs to assess the current referral process pathway and develop a bespoke information system for overseas visitors seeking NHS assisted conception services, because some patients may not be eligible for this specific treatment on the NHS, in accordance with the Guidance for NHS service providers on charging overseas visitors in England.

The challenges

How might we change and improve the efficiency and effectiveness of the current NHS e-referral fertility pathway to identify potential overseas visitors accessing the service?
To record, monitor, and investigate visitors stored within a bespoke information system – assisted conception overseas tracker (ASCOT) prior to an appointment being generated by the trust. This new streamlined approach can identify potentially chargeable visitors for NHS assisted conception services as soon as possible, mitigating the incorrect assignment of NHS appointments. Additionally, it aims to increase social value, generating an NHS cost avoidance of those visitors not eligible. It seeks to increase potential income generation as visitors may choose to self-fund assisted conception services.

• Triage of all e-referrals.
• Reduced staff confrontation.
• Robust automated systematic processing, monitoring, and recording of identified visitors.
• Increased appointment availability (social value).
• Cost avoidance/ income generation.
• Efficient (real time) senior management reporting.

What were/are the outcomes?

• Improved patient care outcomes and reduced onsite patient distress if they are not eligible for NHS funded assisted conceptions services.
• Robust automated systematic processing, monitoring, and recording of identified visitors.
• Increased appointment availability (social value).
• Estimated NHS cost avoidance (£1.1M) to date.
• Income generation (£157k) to date.
